What's the difference between Indiana SFC and Illinois CCP for spousal caregivers?
Indiana SFC pays a flat weekly stipend ($432–$800+/week) based on your spouse's assessed level of care — not hours worked. Illinois CCP pays hourly ($20/hr standard, $30/hr OT) for authorized hours worked, up to 56 hrs/week. For spouses providing 30+ hours of care weekly, Illinois CCP often pays more. Does Medicare pay for spousal caregiving?
Standard Medicare does not pay family caregivers. Medicare covers medical care (doctor visits, hospitals, skilled nursing), but it does not compensate family members for providing personal care at home. The exception: the new Medicare GUIDE Program (launched July 2025) provides paid respite for caregivers of dementia patients. For the most robust spousal caregiver pay, Medicaid HCBS Waiver programs (Indiana SFC, Illinois CCP) are the primary source.
Can I get paid by the VA to care for my veteran spouse?
Potentially yes, through two VA programs. The Program of Comprehensive Assistance for Family Caregivers (PCAFC) pays a monthly stipend (typically $1,200–$2,500+/month) directly to the family caregiver of a qualifying veteran. VA Aid and Attendance pays the veteran up to $2,727/month, which can be used to pay a spousal caregiver informally. VA benefits and Medicaid programs can sometimes be accessed simultaneously. What are the Medicaid spousal impoverishment protections?
Federal law protects the at-home spouse (community spouse) from losing all assets when the other spouse qualifies for Medicaid long-term care. The Community Spouse Resource Allowance (CSRA) allows the community spouse to keep 50% of marital assets up to $154,140 (2025). The Minimum Monthly Maintenance Needs Allowance (MMMNA) ensures the community spouse has enough monthly income. The primary home is generally exempt as long as the community spouse lives there.
How long does it take to start receiving spousal caregiver pay in Indiana?
Indiana SFC enrollment timeline from first call to first paycheck is typically 30–90 days, depending on the waiver waitlist. The Pathways Waiver (60+) has approximately a 1-year waitlist; H&W Waiver (under 60) has approximately a 2-year waitlist. Will receiving SFC pay affect our other benefits?
SFC pay is W-2 income to the caregiver — it doesn't affect the care recipient's Medicaid, Medicare, Social Security, or SSI. The caregiver's SFC income may affect eligibility for means-tested programs (SNAP, LIHEAP) if it raises household income above program thresholds. Social Security: earning SFC income builds your own SS record, which can increase your future retirement benefit. Tax: SFC income is taxable; however, home office deductions and caregiver-related expenses may partially offset this. Consult a tax professional.
Can I work another job while receiving spousal caregiver pay?
Yes. Receiving SFC pay in Indiana or CCP pay in Illinois does not prohibit you from holding other employment. You must fulfill the care hours specified in your care plan, but beyond that, there are no restrictions on other employment. Many spousal caregivers who were previously working full-time now work part-time and supplement with SFC pay — often resulting in higher combined income than before.
What if my spouse's care needs increase — can I earn more?
Yes. For Indiana SFC, you can request a level-of-care reassessment when your spouse's condition changes significantly. For Illinois CCP, increased care needs can result in more authorized hours, increasing your weekly earnings.
Is there a limit to how long I can receive spousal caregiver pay?
There is no time limit on receiving Indiana SFC or Illinois CCP pay as long as your spouse remains eligible for Medicaid and continues to need personal care assistance. As long as your loved one qualifies and you continue providing care, the payments continue. Annual reviews confirm continued eligibility. If your spouse enters a nursing facility or passes away, the program ends.
